Checklist: What to Bring to Camp

Complete the camper application by April 16 and submit with $100 registration fee (payable to Forestry Camp) to:

Campers will be notified of acceptance to Camp. The following forms will then need to be completed and submitted to Jean Szymanski at the address above.
- 2010 Permisson Form
- 2010 Camper Health, Section A
- 2010 Camper Health, Section B (to be completed by a licensed medical specialist)
- 2010 Camper Contract
- Over-the-Counter Medication
- 2010 Letter of Acceptance (this form must be returned by May 7)
